Patients: Copy and paste the exact custom essential oil formula name with date from your treatment plan into the box below. Also, include your name or the patient's name (if other than yourself) in the box provided.
Practitioners: In the box below write a list of each essential oil ingredient + the dosage in mL or drops for each. 12 mL max per bottle. If you include more than 12 mL, we will adjust the formula proportionally. For example, “1 mL each of clove, oregano, cinnamon bark, elemi, tea tree, allspice, peppermint, and mountain savory + 3 drops of garlic oil”.
